For me, Apocalypse Now kept me glued to the sofa.(At least my second viewing) The themes are similar, both involve a surreal metaphorical descent into the primal violence of human nature during the Vietnam war. Though in Apocalypse Now, the metaphorical path is a river as opposed to the Ladder in Jacob's Ladder.

Dead End and Soul Survivors I'd add to the list, and I'll definitely second the movies Stay and The Jacket. Pretty trippy films.

"The Escapist" (2008)

I think "The Escapist" is the closest you could get to "Jacob's Ladder". It's not a bad film either.

Finally, someone mentioned "Soul Survivors" - thanks LeisSuit! That movie was clearly heavily influenced by "Jacob's Ladder."

"Lulu On The Bridge"(1998)

"The Life Before Her Eyes"(2007)

Kontroll by Nimrod Antal seems directly influenced by Jacob's Ladder, touching on similar themes of (imagined) heaven and hell, but without being a mere copy.

Eraserhead by David Lynch, it's the biggest trip on the planet, be prepare for it though, don't expect a straight-forward story, It's Art.

Good post until the final two words, at which point I just heard a very loud farting noise.

Just recently came across this site]http://www.blockbuster.com/browse/catalog/movieDetails/17769

They give slightly more accurate recommendations for 'similar movies'
Imbd is great but its true the recommended movies kind of stink.

Check out these:
Johnny Got His Gun (this one is superb!)
The Sixth Sense

Enter The Void (though it's not for everyone--stay away if you're easily offended or an epileptic--there's a TON of strobe effects).

Twilight Zone: An Occurence At Owl Creek Bridge
Carnival Of Souls ('60s version)

and then there's the rip offs:

Hellraiser Hellseeker
Carnival of Souls (1999)-strange to call it a rip off, since the original predated Jacob's Ladder...but it uses a LOT of the familiar shaking head effects).
Soul Survivors
Stay
