A recent movie very similar to this one is "November" with Courtney Cox in the main character.

Well I think the only movie that I know to deal with similar issues (in a very different way) and to be utterly spellbinding is "Fearless" by Peter Weir, that depicts the effects of post-traumatic stress disorder on a plane crash survivor. Like JL, it rises above a simple drama by bringing in deep philosophical issues of fate and moving on to the next world. Starring Jeff Bridges and Isabella Rossellini in great performances - and Rosie Perez, somewhat unglued.

"The Jacket" which is also very similar to this.

The plot of the "Futurama" episode "The Sting" is pretty much lifted from this movie.

All of these films have similar plot structures:

An Occurrence at Owl Creek Bridge (http://www.imdb.com/title/tt0056300/)
Carnival of Souls (http://www.imdb.com/title/tt0055830/)
The Jacket (http://www.imdb.com/title/tt0366627/)

That's just off the top of my head... but I'm sure there are more.

I asked same question and got some great recommendations from others:

Santa Sangre (1989)
The Game (1997)
The Mothman Prophecies (2002)

Can't believe no one's mentioned 'The Seventh Seal' yet ... that film is the grand-daddy of them all.
