Some of the most disturbing scenes in movies:
Scum - the greenhouse rape.
A Serbian Film - newborn porn.
Hobo with a Shotgun - the scene where the children get burned alive on a bus.
Melancholie der Engel - pretty much the whole film.
Threads - the final scene.
The most disturbing and inconceivably violent film I have ever seen, by far, is the French film Martyrs. It really overshadows everything else I can currently think of, and the entire Shaw franchise, films like Insidious etc are a walk in Disneyland compared to it. I am a huge horror fan -though Martyrs is not technically a horror film- but this film haunted me for weeks, partly haunts me to this day, though I saw it years ago.

The murder of the black man in Joe, not particularly graphic, but I found it very hard to watch.

Typically for me, any scene involving the brutalization of children tears me up. I'm sure a lot of people will disagree with my Welcome to the Dollhouse choice, but that is one of my worst nightmares! It would be like my grandma asking me to pleasure myself in front of her)
