Give that man his prize! Gangsta No 1's hacked to death scene and Irreversible's fire extinguisher death are easily two of the most disturbing scenes in cinema history. Everything else is cheap effects and gratuitous.

#1 for me is the tunnel scene in irreversible. Absolutely gut wrenching. I was a quest to watch horrible, sick things, and once I saw that scene, the rape and the beating, I was like "I'm done! lol" I don't need to watch garbage like that. It really did kill a part of my spirit.
#2 - probably the last beating and skinning scene of Martyrs. It was really brutal to watch having seen the whole movie, and caring about the main girl.
A lot of these movies, people just skip to the bad parts, but without growing attached to them throughout the film, it just doesn't pack the same whallop.
Those are the two big ones off hand...
The dog killing scene in Tyrannasaur is pretty tough to watch. Thank god they don't go into detail. Any kind of animal cruelty I can't stand.
It's been a while, but isn't there a scene in Amistad where they throw all the slaves off a boat tied in one long line with a big rock at the end? THAT was brutal to watch.
Martyrs didn't really shake me up, sure it's a brutal film but didn't really have much psychological impact. Same goes for Serbian Film which, despite it's desperate attempts to shock, felt more like a cartoon than a serious film.
The two scenes in Irreversible really disturbed me though. The fire extinguisher scene caught me off guard, it comes without warning, before I even had any concept of where the film was going I was witnessing a hyper-realistic brutal assault and, although I winced with every strike, I couldn't look away.
The rape/beating scene felt uncomfortably voyeuristic too, I questioned myself morally for watching. Like the fire extinguisher I wasn't so much entertained as I was shocked into submission by it. It looks terrifyingly real and seems to never end.
The most shocking scene I've ever watched comes near the end of The Great Ecstasy of Robert Carmichael (http://www.imdb.com/title/tt0459387/?ref_=nv_sr_1) a film which takes a leaf out of Irreversible's book, it is an unflinchingly vulgar and harrowing act which escalates shockingly quickly.
A film which shocked me when I was much younger was the 1990 rendition of Lord of the Flies, admittedly I haven't seen it since I was about 10, and the reviews suggest it's very poor. But the whole concept of children turning into tribal savages is really scary, specifically the scene when the tribe crush Piggy's head with a rock.
Funny what we find scary when we're kids. Lord of the Flies was on TV the other day, and when I saw the Piggy part coming up, I changed the channel. I'm sure it would have been tame, but I just don't like being depressed.
I watched the torture scene of Imprint against last night to refresh my memory, and holy hell that is one god awful scene. lol. Asides from the excrutiating camerawork and sounds...the part that gets to me the worst is the prostitute who is doing the torturing. It looks like she is enjoying herself way too much, with no intention of stopping. Every needle just makes her more and more excited to stick the victim again. So horrible.
Yeah - the disfigured prostitures ganging up on the pretty new geisha and stringing her up and putting long pins under her fingernails in gruesome detail, and then her gums. Ugh...just awful. lol
Another one is Hellraiser 2. In the beginning where the good Doctor gives the schizo who thinks he's covered in maggots a razor blade. The uncut Director's version just drags on and on...with the patient cutting himself all up. Good old 80's special FX were awesome. The first few seconds aren't that bad, but after a while, you're just like "ACK...next scene PLEASE"
Actually...2 videos that are REAL and I could barely get through are:
2 guys, 1 hammer. That is absolutely sickening. 2 kids bashing a homeless guy with a hammer, then take turns driving a screwdriver into him all over. It's disgusting because you can hear and see the homeless guy groan and try to fight back, but the pain is just too much. It's pretty much the worst thing ever since it is real.
The second worse video is the one where the girl is throwing little puppies from a box into a raging river. Holy crap is it awful. I read somewhere that they caught her, but it sucks animal cruelty charges aren't anywhere near human murder charges.
Both of those will make you feel like you just died inside.
Then there's the drug cartel video of 2 decapitations in vivid detail. One with a chainsaw...and one with a knife. Both are absoutely brutal and vile.
I've heard about 2 guys, 1 hammer and even the name appalls me. I'm not morbidly fascinated enough to watch a snuff film, let alone one where a defenceless man is beaten to death by psychotic teenagers. Also, I read that the man wasn't homeless (not that it should matter) he was a recently retired man with a wife and children. Very grim.
I like films that push boundaries and provoke a reaction from the audience, but only as long as living creatures are not unwilling harmed in the process. Any of the above events you mentioned would be too much for me to watch.
It's not the hammer that's nauseating...it's the jabbing of a screwdriver all into the guys stomach. I think maybe even his nostrils...just for no reason other than to be sadistic.
Yeah, I'm done watching horrible things in real life. I think after Irreversible, I can only do maybe 1 or 2 horrible movies a year. Some things just can't be unseen and stay with you.
French Connection II...those hypo needles really do go into arms...ewwww Freaks...the finale Hunchback of Notre Dame (Anthony Quinn)...follows the book, therefore everyone dies...including Esmeralda and Quasimodo.
