I'm surprised no one's mentioned Douglas Buck's Family Portraits. Pretty rough viewing, and it even has a nice little blurb by Noe on the DVD box. Salo was tough to sit through, more so for the overwhelming nihilism than any particular scene. In a Glass Cage is absolutely amazing, and seriously disturbing. You can pick just about any film from Michael Haneke's catalog and find a scene that'll make you wanna crawl out of your skin (or at least shove your head under a pillow) -- genital mutilation/"sex" scenes from The Piano Teacher, cattle gun scene in Benny's Video, every second of Funny Games -- Haneke uses violence so effectively in that it is always upsetting and never the least bit enjoyable or entertaining. And I suppose since we're on his board, we'll throw 'ol Gaspar a bone for Irreversible, featuring the most horrible rape scene ever filmed.
I've seen Takashi Miike mentioned here a lot, but he's got a bit too much humor in what he does to be truly disturbing/upsetting. Visitor Q comes pretty damn close, though!
Amen to everything you said there. Also from Haneke, the family suicide in The Seventh Continent was more disturbing to me than any nipple slice/eye gouging scenes.
Lots of the best (worst?) ones have been done but the wife beating scene in Nil by Mouth and the father raping daughter scene in The War Zone, both acted brilliantly by Ray Winstone (a truly underrated actor) are two horrific scenes.
