Yes it was. Anakin wasn't present at her birth as he was on Mustafar at the time. The next closest encounter was at the of Rogue One when the Princess's ship escapes from Vader's Star Destroyer.

I would assume it was, because she sure acted like she'd never seen him before. No doubt Leia had heard of him for years, but had never actually encountered him that closely until that specific moment in time.

I don't think so. The way they were talking they probably had interactions in the political arena with the Senate at least.
