How did the sister get pregnant? And secound , if she already knew about the box and ended up with it at the end, all she has to do is not open the box. So wtf with that ending ?

There was never an indication she knew anything about the box. I noticed she did high-tail it out of there when she had that encounter in the mirror, but I don't think she thought it was related to the box. She bought it at an estate sale that was connected to the guy who killed his wife at the beginning of the movie. So to her, I believe it was just a decorative box.

Not sure if I understand you question regarding her pregnancy.

I think she got pregnant from the student.

It doesn't say she is pregnant (it also doesn't show her open the box). Maybe she gets pregnant one day in the future and then opens it.

And she didn't know the box housed anything evil.
