First one to die for me was Matt, then the flamethrower guy, then Chris, then Josh and Sam died last. I thought I killed Jessica but she apparently survived. Also, Sam, Chris and flamethrower guy were accidental. All i had to do was not move but I obviously failed.
shareJess, Matt, Flamethrower Guy, Chris, Ashley, Josh and Mike died in my first playthrough. Was quite pleased that I managed to keep Emily alive, especially after such a stressful scene down in the mines, and I liked Sam, so... Gonna try and keep all those I got killed (expect Flamethrower Guy) alive during my next playthrough.

My survivors were Sam, Jessica, Matt, Mike and Josh.
I saved them all on my second play through.
On my third it was Sam and Jessica.
Then I killed them all.
Then I saved Sam, Mike, Chris, Ashley, Matt and Jessica.
Then I saved Emily, Matt and Josh.
flamethrower guy died 1st, then Emily (shot her w/ Mike), Ashley (head cut off by the Wendigo), Josh (eventually became a Wendigo), and Mike in the finale.
Sam, Chris, Matt, Jessica survived.

1) Matt died (hook through the jaw)
2) Chris died (fleeing from the wendigo after the flamethrower guy got killed. He made it to the cabin but *beep* Ashley wouldn't open the door because he chose to shoot her in the head when Josh had them trapped with the saw trick thing)
Everyone else made it except for Josh. He became a wendigo.
