Please don't be stupid. Tattoos are covered easily all the time in Hollywood.

He wore a shirt because he's not in shape any more. He didn't want that showing on screen.

Not in shape? Lmao. You clearly didn't see the three movies he made after this where he took his shirt off. Crazy ripped in The Expendables abs Bullet To The Head.

I saw this in the comments section also, but am still confused. Couldn't Rambo at some point have gotten tattoos since the last time we saw him? I'm not guessing he was out of shape like some have said since he has Traps that would make The Rock & Brock Lesnar envious.

He didn't have his tats yet when he did Rambo. He got them after. There were on-set photos of him with his shirt off when they were putting squibs on him and he didn't have ink.

Also, this movie was trying hard to be more realistic, or actually, hyper-realistic, than the previous two films. Yes, he took his shirt off in the first one, but only when he was being abused in the police station and it served a plot point. The second movie was the one that had him running around half the movie without a shirt, and it was because he'd been taken prisoner.

The third one was just trying to make the second movie but bigger, so they had him with no shirt again. It didn't make sense in the plot so they didn't do it.

He looks huge, but we'll never know cause he never goes shirtless.
