what's up with the bad guy mastubating using decaptivated head

I saw this movie yesterday and was satisfied with the twist .But one thing which is still confusing me is what's director trying to show by the scene in which old bad guy is shown masturbating with the deactivated head.If that bad old guy is all in the psycho killer's mind then what is the meaning of showing that scene.plz someone explain it.
thanks

It's simple really. The director made a really good film about a violent murderous intruder. One that was definitely male and hates women. This scene showed what a horrific violent murderer he was. - The the director added an extra twist which makes absolutely no sense and tried to pretend it didn't completely screw up everything which had come before.

I don't have an explanation but the trivia section had this to say,

When the killer first appears on screen (pleasuring himself in his truck) the human head he tosses out the window is Alex's. Note the dried blood under the nose. The same as when Alex is discovered chained in her bedroom.

Could this be a hint that the sequence of events might be out of order?

We've met before, haven't we?

I don't know where the confusion comes from. It just shows that Marie was already killing prior to the rest of the events in the film. When Alex is bundled into the back of the van, it shows a shot of the vans blood spatted ceiling. Marie has obviously been on a killing spree using the van. So the head out of window did happen at an earlier time.

Oh for sure, it's also thrown in your face with the multiple photos on his rear-view mirror, and the song playing, "à tout les filles que j'ai aimé avant" (for all the girls I've loved before).

I expressed no confusion, simply that that sequences may not be in order.

Sorry. I was directing the comment to the op and others. Regarding the sequence of events, as Marie was locked in a mental institution at the end I never considered the events were not linear. But it's possible.
