what's up with the bad guy mastubating using decaptivated head

I saw this movie yesterday and was satisfied with the twist .But one thing which is still confusing me is what's director trying to show by the scene in which old bad guy is shown masturbating with the deactivated head.If that bad old guy is all in the psycho killer's mind then what is the meaning of showing that scene.plz someone explain it.

It was shocking, but it didn't flow well with the twist of the film that Alex imagined the killer from the beginning.

I agree. This scene, along with the final chase scene where Marie is alternating back and forth between herself and the killer, are definite inconsistencies with the rest of the story. My only thought is the director uses these two scenes to mirror marie's internal turmoil. The decapitated head scene happens immediately after marie become very upset with alex in re-telling a story about being abandoned by Alex for a guy while they were out at a bar (so that scene shows us marie's underlying sexual rage beginning to boil) and the final chase shows us marie mental state, both normal self and killer self are inside her and at that point, she's alternating back and forth between them.

Final answer:

It's her imagining a guy so sick that she can justify the coming events.

Personally, I think the scene represents her unsatisfied sexual feelings. I don't know if this is what the director meant to do but I see it as she is unsatisfied with her relationship with her friend so that is why she "imagines" herself with a necrophilic tendency, satisfying herself in her friend's parent's house, and being the "hero" by saving her friend from an imaginary murderer. To further prove this point is the scene where they are drinking coffee/tea at night. She hasn't found a good relationship because the person she loves doesn't care about her in that way so she has to imagine herself doing "odd" things/saving her friend from someone who doesn't exist. From here you can take it further and explain however you want.
