I just rewatched this and I think the second shot he missed on purpose, but not the first one. The second one is pretty obvious that he missed on purpose, the first one not so much.

His heart wasn't in it. No surprise he missed. Whether or not it was "on purpose" is hard to say though. Good movies tend to "show" but not "tell." It's in the eye of the beholder.

Not sure, but I knew he would. The conventions of this type of film require that the protagonist sink to the lowest possible level and lose all hope before things get better.

Yes, if you paid attention to any of the ancillary stories/themes in the movie (e.g. the racism, classism, etc.) the missed free throws on purpose was obvious.
