For me it's the culmination of the whole movie in the movie theatre at the end.

The scene that you mention with the old man on the bicycle creeps me a little as consequently something similar happened to me.
Three years after seeing In The Mouth Of Madness I was walking home past a graveyard in my town around 01:00 in the morning late night and the it was as quiet as a it would be at that time suddenly down the dark road I heard an eyrie creaking.
The creaking slowly got closer and I could see someone riding a bicycle in the distance slowly moving in and out of the streetlights near the road.
When I could see the person I found out it was the local elderly little person from my town and he rode past me slowly and looked and me then carried on slowly down the road into the distance again.
This immediately reminded me of the scene you mentioned above and it creeped the hell out of me as it took me back to the first time I saw that scene.
Other than that the scene when you see the changes in the painting creeped me a little I love paintings and art and it also reminded me of the painting scene from the Roald Dahl The Witches film which creeped me a little when I was small and was also quite sad.
Definitely the kid on the bike, that scared the *beep* out of me when I was 12, especially since I was the only one watching it. Then last summer, my boyfriend and I got really high and decided to watch this film (I hadn't seen it since the late '90s) and wow, we freaked out big-time! And then watching it sober a few days later, I was really struck by a lot of creepy bits here and there that I didn't pick up on as a kid.
Now this is definitely one of my favourite horror films.

No one is going to mention the creepy receptionist? I love how the two talked about the book version of the old lady keeping her husband in chains and how it showed him later begging for mercy. That scene really haunted me after watching it!

I get HUGE chills up my spine everytime I see the guy come up through the window where they're talking in the cafe, "Do you read Sutter Cane??" Right before he tries to axe Sam Neill...LOVE it LOL

Yes! I'm glad to see I'm not the only one freaked out by the painting. When the receptionist reveals her true nature is creepy as well. And for some reason, when Trent finds he is part of the cover of Sutter Cane's last book.

It's Mommies day" The creepy painting changing as they talk with ostensibly nice old lady at the hotel and the scens that follow it later, the old man on the bike. This movie is probably one of the greatest atmospheric movies ever.
shareI think the scariest scene is right at the end of the movie when Trent is watching the movie from his own point of view and starts laughing hysterically. When I first saw this in theater's the ending just stuck with me as I was still trying to figure it out, possibly the best ending to a horror film ever. Also, the ass-kicking music during the credits pumps it up and makes it so much scarier.

The whole 20 minutes or so when the entire town goes nuts is really scary stuff. The icing of the cake though was the exorcist type twisted head thing, scared the *beep* out of me.

The Bus Ride...it really shows the true power that Sutter Cane has over reality.

Oh man this movie is just one long fever induced nightmare. The only thing it is missing in the realm of disturbing chill inducing imagery is that of a ventriloquist puppet or doll. If it had one of those things this film would touch on everything I find terrifying about the world.

"Did I ever tell you my favorite color is blue?" and then waking up to a blue world. That part always creeped me out. I think it showed how in control of reality Sutter Cane had become, thus starting the end of the world.
