On a side note: I think the most controversial album cover of all-time is The Scorpions Virgin Killer cover.

I won't even post a link to it here. I think it was banned in like 5 countries.

The Wiki entry for the album has it.

Funny how all the controversy over it didn't really erupt until decades later.

I don't remember much controversy about it at the time it was released (though there was some)

I suppose so, I am not sure what kind of reception it got upon release. It was a little before my time... I liked their music from the 80s though.
