It means to be awake (specific in relation to issues concerning racial justice).
But if you're a white person, it's probably best that you don't use the word, at least according to (white) New York Times journalist Amanda Hess: "The conundrum is built in. When white people aspire to get points for consciousness, they walk right into the cross hairs between allyship and appropriation" (i.e. whatever a white person does, it's destined to be wrong).

Admittedly, I use it a lot, but it annoys me. Basically it's a euphemism for 'we can't have nice things anymore'.
Everyone, (including people out of college or higher than blue-collar class) using the F-word in every single sentence, in fact using it so much, that it loses all meaning.
You see, I take all my F-bombs, and store them away for when I'm reaaalllly angry about something *smiles impishly* (I don't get super-angry very often) and then use them when the time is right, and they have the impact I'm hoping for.
