I don't think killing is what makes Michael strong, it's the mental suffering of people, that's why he was ok in 2018 in the psychiatric, because he was feeding of the pain of his crazy mates. Then when he escapes from the bus he starts killing because the mental suffering of someone Who is about to die must be like a feast for him, and he kills so much he becomes unnaturally strong and is able to defeat all the Mob after they beat him. However he is still a human and is badly injured, and after 4 years not killing much people in the sewer he is so weak.
So yes, I think he is a kind of psychic vampire infected by some kind of psychic entity (which also infecta Corey in Ends, btw) which feads from human pain and suffering.
