Why did she die? Was it ever addressed?

To the best of my knowledge it was never explained what Donna from.

I had assumed it was because they couldn't come to a financial agreement with Meryl Streep's agent, but she appeared in the film after all.

I am sure it must have been a combo - how else could you pass on having her as a lead. All they could agree to was here one scene, I am guessing. Probably she said categorically no, and then they convinced her just one quick scene...

But still, story wise I am curious to why she died. I also asked because, perhaps something was revealed in the first one that had missed my attention and would explain it... guess not.
