Where did this "Ivy" chick come from? They killed the two women. I get that. All of a sudden they're searching for a third. How did anyone know she existed, let alone Randy decide to hide her? And how did Mike know to go to her grandparents' home without even knowing her name? He learned her name by finding the card from Bear while searching her room at the grandparents' place.

Kendall found a picture of all three in Rose's room (the original girl who got killed). She was one of the three girls at the sex party and the rich white dudes told the assassins about her.

And how did Mike know to go to her grandparents' home without even knowing her name?

He got the name Rose from the cop he was liasing with or from the hospital, I can't remember which. So, he was able to find her funeral and find out where the family lived.
