Hypnosis. The guy became an expert in hypnosis. That's the literal explanation I could see. Only AFTER he meets the doc's family, he tells him his plan. The doc's wife, however, does not seem affected by it the way Martin wanted, but hey, she let him escape. The guy is a manipulator playing dumb or mentally challenged; and he wants revenge on a person who killed his father for being drunk.

shareMy opinion is that it was all a revenge day dream from an angry child. The clues are the disjointed dialog, the limited knowledge of what a female does during sex (mother and daughter), how the girl fell so deeply in love with the boy and he rejected her.
