We saw the legs and hands on the two dudes- theories on what the thing was on her back? I noticed she always seemed really tired...

Since the very first scene of the movie, they are in the simulation. Hayley has an implant that allows the aliens to lead the pace and course of the experiment through her. The experiment to get nic to accept his legs without going crazy from insanity or invincibility hinges on how he reacts to Hayley whose actions are of course led by the Aliens.
