The PO told us Alex (the brother) died in a prison fire, which was clearly to get us to think "oh Alex must be in his imagination." But it turns out Alex wasn't his imagination. So I guess it was a red herring. But was the PO lying to him about the fire? Or did she really think the brother was dead?

I wondered the same thing. I think the writer was more bent on us thinking it was Alex's imagination than being clear about whether she knew or not.
