I suppose I might have missed it in skimming 6 pages, but it looks like no one's mentioned David Lynch's "Eraserhead," one of the more surreal and nightmarish films I've seen to date. The slugs and wormy critters and stomping them is clearly a nod to similar stuff in "Eraserhead," if yall remember the Lady In The Radiator...

Thanks to the commenter who brought up Lynch's "The Grandmother," never heard of it but I'm watching it now...reminds me a bit of The Residents' short surreal film "Whatever Happened to Vileness Fats?"

I recommend Banshee Chapter (2013). It's not as quirky and colorful, but has a similar premise: People take a mysterious drug and start seeing horrible things hunting them. Some parts are found footage-y, mostly flashes of archival footage and whatnot, other than that it's a regular movie, and quite scary. There's also a great character sort of based on Hunter S. Thompson, played by Ted Levine (Silence of the Lambs). It has a really intriguing premise and draws from a lot of creepy subject matter.

If you liked John Dies at the End as a sort of modern take on H.P. Lovecraft, you should really see Banshee Chapter.

It's not as funny, and not particularly good, but The Attic Expeditions had some similar themes and feelings I think.

This movie kept reminding me of "Repo Man" (1984), at least equally awesome.

And, more recent, "Resolution" (2012) by Justin Benson and Aaron Moorhead, very much worth watching.

I know this thread is old by now, but I'm just gonna throw out a title
Dellamorte Dellamore (1994) ...Cemetery Man
... It's the movie I was soonest reminded of when watching John Dies.... The surreal shifts between reality and... something else... The humorous non-chalant violence. Quite similar in structure even if Cemetery Man is a little more dark and philosophical and arty. Still, similar.
