Why was that guy's voice so messed up?

Did it ever explain why the main villain's voice was so messed up? If so, I must have missed it, and it bothered me throughout the entire movie. Why make a main character's voice messed up without an explanation? It was a terrible distraction from an already convoluted story line.

He's supposed to be reminiscent of all those Insane Emperor types we've seen in other works, like Nero or Caligula or the truly bad portrayals of George III.

This. Even most of the names were Roman (like Titus, Famulus and Jupiter) or Roman-inspired. It sounds like he was channeling a bit of Joaquin Phoenix as Emperor Commodus in Gladiator. Likewise, the set design and decorative style of Balem's massive soul-industrial complex and palace was reminiscent of the imperial buildings in Gladiator and other sword and sandal epics.
