he was terminally ill, so these folks saved him and made him immortal, only catch is, he looks like crap.............. so the rest of the movie is just him killing them one by one.... for saving his life, but making him ugly!!??

I guess you missed the part where they brutally tortured him for weeks and were always intending to make him their slave had he not escaped.
