Incest did initially cross my mind at first when watching this several years ago, but technically they're not blood-related and I don't recall Mel mentioning how long he was married to Josh's mother. Maybe Cher didn't interact much with Josh as a typical biological brother and sister would knowing each other from their very early childhood years.
Although, it's not going to stop those future family Thanksgiving dinners from being a little uncomfortable.

He might treat like a son but it isnt his son. It happens all time where you treat friends, elders, or younglings as your kid, parent, mentor, or brother. Its all meant affectionately in admiration. Has nothing to do with sex.
