So why is it gross when an old lady says the thing about Miles Davis?

I've only seen it that one time, when it was new. I didn't like it much, so I never rewatched it. I don't remember that scene at all, or any joke about Miles Davis.

Well, the joke is that Miles Davis epitomizes cool, so the lady is saying that she pees her pants all the time. The humor lies in the self-deprecation. She's admitting to doing something embarrassing, and saying "if this awkward embarrassing thing were actually cool, I would be considered the coolest person in the world, because I do it all the time."
It sounds like she was saying it to console the kid who wet his pants. It's gross for an obvious reason-- she's admitting that she is incontinent, and that she is always peeing her pants.
