Did the book say that. Where's Connie and Vincent at the time of his death. There would of been by his side the same with Al Neri.

They wouldn't have been by his side if they didn't know he was going to die. He wasn't exactly in a hospital. And it mirrors the way his father died...without his immediate family "by his side."

We don’t really know whether he got alienated by his family and died alone. The movie suddenly jumps from Mary’s death to the final scene

Some people say the final scene means Michael died all alone, estranged from the others

But on the other hand - he admits that he loves Sicily, so he might’ve been living there by choice. why would his family be around him at that specific time: you can’t know exactly when he is going to fall down and drop dead
