I know this is gonna sound incredibly cheesy, but I interpret the ending as them dying (which is kind of obvious) & going to heaven. Please note: I'm not even religious myself, but that seems to be what the visuals of two patches of light ascending into grey time-lapse clouds that break into a sunny blue sky at the final moment imply...

Up til now I'd always assumed they died at the end of the film; reading people's ideas on here I wonder if they actually died in the initial bomb blast, but only realized as much once they'd actually recognized the hopelessness of their situation (a bit like a certain famous supernatural thriller?).

I don't see any reason to assume they died any sooner than the end of the film. Otherwise why hallucinate radiation symptoms that they wouldn't be familiar with and obviously didn't pick up on as being symptoms of acute radiation poisoning? If they had died right away, they wouldn't have experienced those symptoms as they'd have been dead before they presented and they were both quite clearly too clueless to imagine those things happening accurately.

And of course even at the end of the film they may still be linger on for a bit afterwards: it only really matters that they might as well be dead and certainly will be soon if they're not already, it doesn't really change the message either way. Bottom line is that dying of radiation sickness as part of a nuclear holocaust blows chunks and we really ought to avoid putting ourselves through that as a species if we can, whether the film ended with them dead or 'merely' near death. But treat the whole thing as a kind of dream or imagine afterlife scenario? Well, shoot, maybe the reality won't be quite as bad as all that, so let it ride!

You're right, my idea doesn't scrutiny as the couple's reaction shows they had no idea what would happen in the aftermath, bless them.
