Where did the red pillow come from? DId he have it? Why did he give it to trolly driver??

"Stole it from a whore house in Creed." He sat on it to relieve the pain when sitting (as on the trolley). He gave it to the trolley driver because he wouldn't be needing it anymore (he intended to go to the saloon to die).

There was the scene with the doctor where he said he got it from a whorehouse. The red pillow helped demonstrate his rough, wild character. It also provided the moment when he gave it away at the end because of his upcoming demise.
