On "One Tree Hill" it was 20 years later but how does the shape of her entire face change? How is this even the same person?

She probably had a face lift, lets all mock her for doing it.

Her chin got pointier and her nose got wider. A face lift wouldn't maker her nose wider.

Different hair style framing her face. Different makeup style. Age, and some weight gain.
