My wife doesn't and I don't really know what I have one for.
I have an over 10 year old smartphone that has cost me 199€ back then and I got that only because there was a discount offer for a contract with unlimited phone calls in all of Europe and the US plus 30GB traffic in 4G for 5.99€ a month, guaranteed price forever. I just couldn't refuse that.
In practical use I use that phone to make international phone calls for free, which I do less than once a year and for access to services that require a digital signature, such as the national health service here in Italy in case I'm too lazy to drive to the doctor for a prescription.
That's it, effectively I'm not even making enough use of it to compensate the 5.99€ a month.
When we leave the house I leave the phone at home, I have no use for it, because for emergencies my wife has a prepaid regular mobile phone (without smart) that costs her less than 5€ a year only because that's the minimum recharge one has to do per year to keep the number active, she has like 50€ credit on that phone because she never uses the 5€ she has to put onto it per year.
When people ask me why I don't use it more I usually answer, I don't see why I would use a 6.7" screen while I have a 75" 4K TV I use as monitor on my desktop PC that has 1gbit internet.
There just isn't anything I might want to do on the web while I'm outside that cannot wait until I get back home.
