"When the wind blows" (1986). Its old western animation dealing with an old couple who dont quite get what nuclear war is. I know its hard to find but you will be hard pressed to find one sadder than this.

Watership Down (Bright Eyes) and Dead Poet's Society (Oh Captain My Captain) always bring me to tears.

The Boy In The Striped Pajamas also was heartbreaking.

Not a film but I only have to think about the last 6 minutes of Six Feet Under and I am away...
