What’s with calling people the wrong name? It’s done at least twice throughout the film.

I think you’re confused, were is the incestuous relationship bit coming from.
Lucious says that his father is Lucious Verras or something, but it’s implied that his mother and Maximus had a relationship at some point, maybe even during both of their relationships with their respective partners. Either way you spin it, there was no familial relation between any of the people involved, aside from Lucilla and Commodus, and it’s made clear that she was not hooking up with him.

Kauffman was an artist, and a troll and did what he wanted, and should be respected for staying true to himself and not selling out like the majority of people do in Hollywood.
Just on the cheeks maybe a few times I think, as they do in that part of the world in greeting and so forth.
