Was he stalking the doctor and wife out of revenge & bitterness for his lost youth as he was innocent or was he guilty of the childhood crime and just psychotic?

I don’t think the writer wanted to make that clear. It certainly wasn’t to me.

I think it was pretty clear he was a psycho. The doctor realized this at the end, and he realized he wasn't going to stop. That's why he drove him off the bridge.
