Why did Cindy (the woman who allegedly killed the blind man's daughter didn't remove her gag ? Her hands seemed to be free.

i thought her hands were still bound but i could be wrong.

How did he get her in the first place?

Her hands were clearly free. I do not know how he got her and we can certainly not tell that from the movie.
