Can someone remind me....(SPOILERS)

I read the book years ago but I can't remember everything from it. I obviously remember how she freed herself and the Moonlight Man. But there's a few things I can't remember were actually in the book or not -

1. In the book, I remember the voices, but I can't remember who the voices were or what they represented. Were the voices just herself and her husband too? I thought they were of other people she knew.

2. Were there flashbacks in the book, and were they about her and her father? I honestly don't remember any hint of it.

3. The ending in the book was that after everything she went through trying to escape, she ends up dying in the car crash. Is that right? If not, is the ending in the film the same as in the book?

4. Was there a dog in the book?

Sorry, I know it sounds like I may not have paid attention to the book at all but I only read it once and it was 10+ years ago lol.

2. There were flashbacks in the book and her father definitely did something sexual with her on his lap during the Eclipse. I'm pretty sure the movie was pretty close in this regard. I read the book awhile ago though.
3. She doesn't die in the car crash in the book.
4. There was definitely a dog and he ate the husband. The book even mentions the dog eating the "forbidden meat" because it was so hungry or something along those lines.
