Considering events continue on in the post-apocalyptic reality even after he dies in the one where he's institutionalized, maybe it's more than a figment of Reggie's dementia-ravaged brain. Remember, Mike was once in the position Reggie is in here, institutionalized because nobody believed his stories of a supernatural mortician and his army of dwarves and spheres. Then of course, Reggie's family was blown up and it all turned out to be real.
