Why did Mrs Sprot call Tommy "Mr Beresford" at the beginning of this episode considering she only knows him under the name of George Meadows? Is that a goof by the actor or some sort of hint?

Yes, it was a goof by Christina Cole.
And minutes later, the psychiatrist calls him Tommy.
