Final Song at the End

As Cosmo & Raphina are on the boat together at the end, is the song that is playing the one that one his brother wrote for him? Before he gets on the boat his brother hands him the note pad that and says it's a song about a guy who falls for the girl and the future. The final song is clearly about following your dreams now and you must go now, which tells me this was the song the brother wrote. Would I be correct?

This is how I interpreted it, and whether or not it's true, it's how I'll continue to look at the scene. Imagining that those are the lyrics his brother wrote makes that scene even more poignant. It's already a scene I cry at every single time I watch the movie (up to five times now) as the final embrace of the two brothers just wrecks me, as well as Brendan telling Raphina to look after him jokingly as he'll be lost without him, but you then immediately see in Brendan's eyes that he'll be lost without having Conor around to talk to through all the home drama. I loved the relationship between Conor and Brendan, such a great plot point and Jack Reynor did a fabulous job and I think interpreting that final song to be the one Brendan wrote (probably intentional) makes the movie end so beautifully. Love it to pieces.

Yes you are! I Just read the screenplay, I remember this question of yours, and it is confirmed in the Original screenplay Text. I pasted it below and put in the link for you to read on your own if you like.

EXT. THE BOAT - CONTINUOUS
Conor waves up at the people on deck. Then speeds onwards.
Raphina smiles, looking at his back. She tries to light up a
cigarette. Then gives up. She starts laughing.
The SONG really takes off now. It’s Conor’s voice singing.
The lyrics are about him and Raphina. About their future. A
model and a singer. About staying together. Written from an
older brother’s perspective. What we’re hearing is clearly a
marriage of Brendan’s lyrics and Conor’s music. From the
future. And it’s the only clue to whether Conor and Raphina
ever made it. Because right now, all we can see is mist,
rain, and grim determination on COSMO’S drenched face....
Snap to black.
