I think I'd safely say it's a prosthetic. Not only because it just doesn't really look all that real to begin with, but also because the MPAA has a ridiculously intricate outlook on how the male "package" can be handled, so to speak, or viewed in a movie. Even down to the way it's moved around on the screen. It looked like the one from the movie "Step Brothers", to be honest! Here's an article about it if you care to read...

Sascha sucks poison out of Mark's balls... Although it's fake balls obviously... But yeah alot of shots of that

Also alot of Elephant dick and vagina (in which makes the perfect hiding spot according to Sascha's character) so if you're into stuff like that than this is the film for you
