the treatment process of Claeren

From what I gathered, Claeren suffered from multiple personality disorder, his alter ego firmly believing he was the next step in human evolution. He was also under the delusion that the female hormone prolactin was a toxin attacking him, making him impotent, and thus preventing him from reproducing and passing on his genes, thereby thwarting the next step in evolution.

That is why Claeren would remove all milk from the household he intended to siege (prolactin is associated with female mammalian milk production) and that is why he attempted to disinfect areas he thought were possible sources of prolactin (peeing almost everywhere). That is probably also why he kept peeing at the door of the room where the last victim's mother was held (to prevent her prolactin from coming out and attacking him while he underwent his treatment).

Claeren was also attracted to children, and believed that they were they key to giving him back his potency. Since he could not rape the children himself, he forced the childrens' fathers to do it for him whilst their mothers were imprisoned elsewhere in the house. As the fathers raped their own children, he masturbated to the sight in an attempt to regain potency, but to no avail, which is why he continued to search for new victims incessantly.
