I don't really think that the affair was a central part of the plot in Leviathan. The affair happened only to show the viewer that Kolya's wife was so desperate to escape the grim, depressing life in the neck of woods of the Russian north, that she would be willing to sleep with her loving husband's best friend in order to escape that pathetic life.
