I think the point of the scene was that we think he has missed her with the blade when he kicks out at her, and that she has the upper hand, then we realise at the same time as her that she has been mortally wounded. It was quite well done, I thought.

Klebb is not the only character to use a poison tipped dagger hidden in a shoe. Morzney kills Kronsteen with one when his plan fails.
