I'm wondering what happened to Dr Hill? Was he in Josh's lair underneath the lodge or just in Josh's brain?

It was all in his head, just like the weird stuff he was seeing in the caves.

Not entirely true...the Dr. Hill we see in the game is a hallucination spawned by Josh's subconscious, but there IS a real Dr. Hill. If you fully explore the basement before the reveal that Josh is the 'killer' you can find a letter from Dr. Hill (he was the one who treated Josh after Beth and Hannah disappeared).

Dr. Hill was the shrink Josh was sent to after B&H disapered. he kept going back int the sessions through out the game. At the end, he is lost in a cave and realized that he is left on his own.

Well, considering you always see him in his office up until the point that Josh is lying in the mines where Dr Hill and his desk are, I'd say he's clearly in Josh's brain. I mean, unless you're suggesting that Dr Hill somehow carted his desk and table lamp down from his office to the mine, just for that?
