Pablo Escobar wasn't a mafia boss.
Colombian drug trafficker.
Big difference actually. Mafia usually equates with Italians. Oddly enough, mafioso bosses have become such a mainstream part of society that they themselves look to films sometimes to decide how to present themselves to the public. John Gotti was very much aware of his public image and tried to present himself as suave and sophisticated.
In this way, the public's demand for a top mafioso to look a certain way actually determines how one looks.
I don't know what sort of social pressure Escobar had to look a certain way but whatever it was I think it had a lot less to do with the US public, Hollyood or Italy or how mafiosos look on US television shows than his own culture.
He wore shorts a lot. I can imagine this is more to do with Colombia's climate than living up to an image so a US viewer will understand what he is by studying his attire and expressions.
