I was surprised by that too, but I think it was implied that it was Billy and his men - not to say that makes it less bad. When Flint and Madi left they were alive from what I could see, and in this ep they said something about Billy being in control of the Underhill plantation.

Yeah, it was Billy's men. Flint is capable of doing that, too.

And I suppose we can say Underhill and the other plantation owners were willing to kill women and children, too.

This entire series has been about the monsters that men become in their prosecution of the Wars they're are called upon to fight. We saw this most with Flint (esp. after Miranda was killed). We're later shown how Silver becomes a man willing to be brutal--quite an evolution from his beginnings. We see this also in much lesser degrees with Vane. On the other side, we see it from Rogers and Berringer too.
