No, the first dumb words that will come out of your helpless mouth are "Dude, are you okay?" You know they're not okay, but if they answer to you then it means you guys can probably handle it.

Exactly this. Cliff was more terrified of what was happening to Derrek than Derrek was, it makes sense that he doesn't know of any way to help his friend, but he's hoping to hear a "Yeah dude, I'm okay".
It helps to think about it backwards, the Derek and Cliff (in real life) probably thought Cliff needed to be saying something while filming Derek, and without any realistic advice to offer, there isn't much to say beyond "Are you okay?" and "What the hell?"
I knocked one star off the rating just for that. Annoying as hell, and repeated ad nauseum. Derek, are you ok?? Ugh!
