Becoming an Upir? can someone clarify something....(finished season 3)

I haven't had a scroll through these boards so not sure if this has been asked and answered yet but ill fire away..

I was lead to believe due to revelations in past seasons etc that you become Upir when you a) have the foundation of being born with the 'cowl' and b) willingly commit suicide.
Olivia turned because she killed herself trying to chop off her tail when she was a child and Roman when Olivia manipulated him into committing suicide through a cruel well thought out plan of secretly forcing him to sleep with his own sister, birth a baby with said sister and unsurprisingly hate himself to the extreme limit; Roman becoming Upir being her ultimate plan.

So, my question is, what's with Annie? the only insight we get into how she 'died' was when she states that she had a family, husband and daughter once, and after a car crash she became Upir. I might be remembering incorrectly but I'm sure she was implying that this is how her 'new life ' started. This may have been just a bit of dialogue of how she's lost her loved ones etc but wasn't it the case that she was explaining how she became Upir? If so, this wasn't suicide?
Or again have I remembered incorrectly and I'm making the whole 'suicide results in Upir status' up!?

Any thoughts ppl!!?

When Annie shows the pics of her daughter and husband to Olivia they are modern pics. But Annie has been a nurse since the early 1900s. So the car accident was just an unfortunate incident but unrelated to her turning upir.

Not sure if it has to be willingly.

I know it was the caul, but the suicide was never explained. We only had Olivia's flashback and Romans experience to reason that it was a part of becoming an Upir.

Yea seems like a bit of a plot hole, as if the caul and suicide are both answers for upir.
