I probably missed this, but who was the guy at the bar with someone while Sam was having a drink at the restaurant. He slipped her something then left. Was that BJ and why slip her what looked like a twenty?

No, that was not BJ. Sam's vision was going at that point; she was delusional.
The $20.00 was for his drinks. He was giving it to the bartender to pay, since the bartender was standing right in front of Sam.
I don't think that Sam's vision was going, I think it was just a cheap ploy by the director to keep his identity hidden. Like it mattered. Her shades remained on the entire time (even when she somewhat chased him). She had no trouble moving about before or after that scene and she had no problem violently attacking two people, biting a neck, applying make up and driving.
