It would be difficult. For starters, 3 of the survivors actually died. If they are still in movie land. Then the 3 shouldn't be there. The only way to do a viable sequel would be to use Hollywood magic and erase the last 120 seconds of the film. I guess open up part 2 exactly where part 1 ended. Add a few minutes of running from/fighting with the killer. Then boom... Everyone wakes up or something. Then part 2 can be whatever. Personally I would go with having the characters from the movie coming out into the real world.

Actually Max and her group of friends weren't originally in the movie and therefore weren't created and scripted to die in that movie so the fact that their deaths weren't permanent doesn't bother me that much. I actually figured something like that might happen with the way the start of the slasher movie kept resetting after however many minutes until they got into the van when they first got sucked into the movie. Also Duncan popping back up seemingly ok after getting hacked by Billy (unfortunately just in time to get smucked by Paula's car) was another small hint that their deaths might not last. I just figure "hey magic" because seriously? This is a movie about a group of kids that get sucked into a movie lol.
If they did do a second movie it could be kinda interesting though if it turns out that because Max killed Billy and survived, she is now an actual part of the sequel. She has taken over the character of the official final girl and cannot leave the movie world. Maybe she finds herself saying some of the lines that the original character said, and wanting to preform some of the same actions that character did without really knowing why. It could be an unconscious thing, like she feels pulled to behave that way but if she concentrates she can still control her own actions. At first her friends wouldn't realize it because it would be just a bit odd but nothing that raise alarm bells until she does something completely out of character. The second movie could be about them all trying avoid being killed and finding a way to bring Max out of the movie with them at the end. Then the final scare tag of the movie could involve them finding out they escaped but also brought Billy with them.
...and in conclusion that is why squirrels should always wear pants.
